At least 18 years old
Free from signs and symptoms of any communicable disease
Must be able to perform ADL’s with supervision or assistance
Must be able to transfer with assistance
Must be able to be assisted with self administered medication (special provisions may be provided by a nurse for those who must have their medication administered)
Special dietary needs must be able to be provided
Must not be a danger to themselves or others
Shall not require 24-hour nursing supervision or mental health treatment
Must not be bed ridden and once admitted, must not be bedridden for more than 7 consecutive days
Must not have any stage 3 or 4 pressure sores
Must not require: assistance with tube feeding, oral or nasopharyngeal suctioning, monitoring blood gases, skilled rehab as outlined in 59G-4.290 or treatment of surgical incisions unless it is stabilized and is accompanied by a plan of care
